1) Click on the program you are registering for below and fill in the pertinent fields. You will need to fill complete this form for each learner… then hit the ‘SUBMIT’ button at the bottom.

2) Complete  and sign both of the following: Medical Release Form & Photo/Video Release for each learner. These forms are due on the first day of class. You only need to complete these forms once each calendar year. EVERYONE must complete a new form for 2012. You can always complete these forms on the first day of class if you arrive early.

3) PAYMENT. We cannot accept payment online at this time. All checks should be made out to ‘Studio BE, NFP’. You can either schedule an appointment with Mr. Aaron (aaron@studio-be.org) and drop off your payment, or you can mail your payment in to Studio BE at 3110 N Sheffield Ave, Chicago, Il, 60657.
